Reporting Guidelines

For

Nonappropriated Fund (NAFI) Employees

Of MCCS

 

  1. If employee perceives injury or illness is job-related, employee should immediately report incident to a supervisor.

 

  1. Supervisor should fill out top portion of Occupational Health Permit Form (see Human Resources / Forms, this website) and give to employee to take to Occupational Health Clinic, Building 65, if medical treatment is necessary.

 

    1. Occupational Health Clinic, Bldg 65 is open Mon thru Fri, 0700 to 1500 hrs
    2. Naval Hospital Emergency Room is available for NAFI employee job-related emergency treatment Evenings, Weekends, Holidays or if EMT is called.

 

  1. Supervisor should fill out Flash Report (see Human Resources / Forms this website), after ensuring employee receives medical attention, if needed, and follow instructions for sending form forward.

 

  1. If incident reported by employee is first aid only, no loss of consciousness or no loss of time, a Flash Report should still be processed rather than the Incident Report forms previously used.  (Note: The Flash Reports assist in identifying potentially hazardous situations which may help to avoid future accidents and injuries.)

 

  1. When an employee has been written off work by a medical doctor, their work status note should be forwarded to Human Resources (fax 451-6776) as well as advising HR of the date employee returns to work.  This is so that proper notification can be made to Department of Labor.  Please call 451-2047 or 451-1884.
