TA REQUEST FORM
This form is for use at Camp Lejeune and MCAS New River ONLY! It must be typed. If you have questions on how to fill in the different fields, please contact our office.

SAMPLE LATE SUBMISSION WAIVER LETTER
Occasionally mission requirements force students to miss the TA submission deadline.  Students may submit a Late Submission Waiver Request due to the circumstances listed below.  The request must be made to the Education Service Officer (ESO) within five days after the term start date or it will not be considered.

  1. The student missed the submission deadline due to a MILITARY COMMITMENT or an ADMINISTRATIVE ERROR due to their command.
  2. An example of the late submission letter is available from the form link or from our office.

SAMPLE REIMBURSEMENT WAIVER LETTER
If a student voluntarily withdraws from a course resulting in a “W” grade and the cost of the course is passed along to the government by the institution, reimbursement to the Marine Corps in the amount paid will be required before another course can be taken using military TA.   Reimbursement may be waived if the withdrawal was duty related.  However, military TA policy states that duty waivers will not normally be met with a favorable endorsement.

Requests for waivers of reimbursement must be submitted through the military education center where the student received TA.  This formal request must be written and signed by the student’s Commanding Officer and forwarded to the Education Service Officer (ESO).  Supporting documents (orders, medical documentation, etc.) must accompany the waiver request when it is submitted.

Students should complete this form if they want to start utilizing their Montgomery GI Bill (MGIB) benefits.  All students should read over the current regulations for MGIB on the VA website at www.mgib.va.gov
